Because every person has uniquely different needs when it comes to therapy it is best to schedule a consultation. This way someone on the SG Mental Health Counseling team can meet with you to assess your goals and concerns, and collaboratively arrive at the best course of treatment for your needs.
Everything on the client intake form can be filled out except the signature area. You will need to print the document after filling it out and sign your name. Please bring these documents to your first appointment.
